ASTRO Guideline Focused Update on Radiation Therapy for Rectal Cancer

This guideline update addresses three key questions focusing on the appropriate indications and dose-fractionation regimens for neoadjuvant therapy, optimal sequencing of radiation therapy and systemic therapy in the context of total neoadjuvant therapy, and considerations for selective omission of radiation therapy and surgery for patients with rectal cancer. All recommendations are either new, modified or unchanged from the 2021 guideline, except for the KQ4 section which remains unchanged as there were no new practice-changing data. This guideline is a focused update of ASTRO’s original 2021 guideline on radiation therapy for rectal cancer.
