This form is just a tracker and cannot be submitted to earn credit. To claim credits, you must complete the online evaluation.

CME certificates of credit are only available to participants who have completed the online evaluation tool. Participants will have online access to their certificate upon completion of the evaluation.
Physicist attendees will not receive certificates of credit from ASTRO. The names of physicist attendees will be sent to CAMPEP 45 days after the meeting. Any credits claimed after the evaluation deadline will not be submitted by ASTRO to CAMPEP.
