Registration Deadlines
The last day to receive the early-bird discount is August 14, 2009. Registrations received after this date will be charged the advance rate. The last day to preregister before the meeting is September 4, 2009. After September 4, 2009, you will need to register on site at the meeting.

• Early-bird - on or before August 14, 2009.
• Advance/On site - after August 14, 2009.

Virtual Meeting - New This Year*
Extend your learning experience with access to the symposium sessions after the meeting. You will receive streaming content that has been digitally recorded live and published as audio synchronized to PowerPoint presentations. You are able to purchase the Virtual Meeting when you register. Please refer to the chart above for rates.

If you purchase the Virtual Meeting, you will receive an e-mail message approximately eight weeks after the meeting which will provide you with a link that will allow you to access it.

Registration Confirmation
Individuals who register online will receive an instant e-mail confirmation. If you fax or mail in your registration, your registration confirmation will be sent via e-mail within three to five business days.

Registration Cancellation Policy

• Refunds will be given if written notification is received on or before August 14, 2009.
• All refunds are subject to a $100 processing fee.
• Telephone cancellations will not be accepted.
• NO REFUNDS will be given after August 14, 2009.
• Registration fees are nontransferable.
• Registration refunds will be processed 30 days after the conclusion of the meeting.

Visa Information
Citizens of foreign countries, in most cases, will need a visa to enter the United States. It may take up to three months to obtain a visa. You must be registered and paid in full before you will be sent a visa letter of invitation.
