All Subcommittee members will complete standard
ASTRO Conflict of Interest
disclosure reports on an annual basis.
Prior to project implementation, all Task Group members and ETC members will complete task specific Conflict of Interest Disclosure Forms for individual projects on which they must indicate any and all real or potential conflicts related to the particular device or procedure. Any individual who has not yet completed the standard ASTRO Conflict of Interest form will also do so.
Copies of the signed Conflict of Interest Disclosure Forms will be sent to the ETC Conflict of Interest Primary Reviewer.
The primary reviewer will review all Conflict of Interest forms, and will identify and resolve any potential conflicts of interests as instructed in the ASTRO Conflict of Interest Policy, Part IV, making use of the Conflict of Interest Review Committee as necessary.
For purposes of ETC Conflict of Interest review, a chair of the ETC will be deemed the primary reviewer. With respect to any conflict of interest issue involving a chair of the ETC, the primary reviewer shall be the ASTRO Conflict of Interest Review Committee.
If in any instance, both ETC co-chairs are recused from a particular project, a primary reviewer shall be appointed by the ASTRO Board or by the chair of the ASTRO Conflict of Interest Review Committee.
