1. Please read the ASTRO Fellow Nomination and Selection Process for eligibility and other criteria in its entirety.


2. Please complete the entire FASTRO nomination form. You can use the online version and submit your application electronically, or you can download, print and complete the form to be mailed to ASTRO.


** Due to the length of the application, nominees are strongly encouraged to submit their application and all supporting documentation together by mail, FedEx, UPS or similar mail delivery.

3. All sections of the application must be completed or the application will not be accepted.


4. Required supporting documentation and letters include:
   
• One letter of nomination from approved list.
• Three letters of support from the approved list. Only one letter of support may come from a nominee’s business partner.
• A current curriculum vitae.

5. Please note that letters of nomination and support can be sent directly from the nominator or supporter to ASTRO. However, ASTRO prefers that all letters of nomination and support be attached to the application. Candidates choosing to have letters sent directly to ASTRO are responsible for ensuring they reach the ASTRO offices by the submission deadline of March 31, 2008.


6. Completed forms must be signed by the nominee or they will not be accepted.


7. All applications, including all supporting documentation and letters, must be postmarked by March 31, 2008, in order to be considered for 2008 ASTRO Fellow status.

ASTRO 2008 FELLOW NOMINEE (FASTRO)
INSTRUCTIONS FOR LETTERS OF NOMINATION OR SUPPORT

1. Nomination and support letters must be from eligible ASTRO members. ASTRO members eligible to nominate or support candidates must be from one of the following groups:
   
   a. Current or former members of the ASTRO Board of Directors.
   b. ASTRO Gold Medalists.
   c. Current or former departmental chairs.

2. When providing letters of nomination or support for a FASTRO nominee, the letter must outline the nominee’s contribution to the field of radiation oncology and clearly establish the scope and significance of the nominee’s accomplishments.
   
   
3. Letters of nomination and support are limited to two pages.
   
   
4. Please note that letters of nomination and support can be sent directly from the nominator or supporter to ASTRO; however, ASTRO prefers that all letters of nomination and support be attached to the application. If the nominee has requested that your letter of nomination or support be sent directly to ASTRO, it must be postmarked by the submission deadline of March 31, 2008.
   
   
5. All letters of nomination and support must clearly indicate the candidate’s name at the top of the letter.
